College Counseling & Standardized Testing FAQs

• Parents need to submit a signed copy of the Consent to Release Information to ACT form (listed under Stage 1) to the College Counseling Office.
• A current individualized Education Program (IEP) or Section 504 Plan (504 Plan) that documents accommodations routinely provided in similar settings, such as in academic classes during instruction and in other testing situations.
• To qualify for accommodations in the absence of a valid, current IEP or Section 504 Plan, an examinee must submit documentation that identifies a professional diagnosed physical or mental impairment that substantially limits one or more major life activities AND requests allowable accommodations that are appropriate and reasonable for the documented disability. The documentation must be reflected in a letter or report from a licensed, qualified professional, and complete diagnostic documentation may be required in some cases to substantiate a need for accommodations.
• If you are submitting documentation that is NOT an IEP or 504 plan you must ALSO complete the Exceptions Statement Form (listed under Forms) and submit to College Counseling.

The school official is responsible for submitting requests for students who need ACT accommodations. This includes the diagnosis and the consent form. Click here for more information.

• Be sure accommodations are needed. College Board tests are administered in a structured, quiet setting. It is important to consider how your student functions in this type of environment. Accommodations appropriate for school or home may not necessarily be needed on a College Board test given the quiet, structured environment.
• Start early. Know your dates and deadlines. Services for Students with Disabilities (SSD) review every paper request and accompanying documents thoroughly so processing can take up to seven weeks.
• Fill out the Student Eligibility Form and include The Meadows School College Board High School Code (290116). Read these Student Eligibility Form Instructions carefully. When a family sends the Student Eligibility Form and documentation to the College Board, all sections except Section 16 (Confirming Information and Signature) must be completed. An incomplete Student Eligibility Form will not be processed. Documentation is always required from families.

• The diagnosis clearly stated.
• The information is current.
• The student’s educational, developmental, and medical history are present.
• The diagnosis is supported.
• The functional limitation is described.
• The recommended accommodations are justified.
• The evaluators’ professional credentials are established.
